Choose a patient or client whose care you have been involved in whilst in practice. The patient or client should be an individual whose health and wellbeing has been impacted by psychosocial factors. These may not be the main presenting issues for the client but will have impacted on the client’s health and well-being.Please ensure you discuss your chosen patient/ client with your facilitator before writing the assignment.
