Preparing the Discussion
Follow these guidelines when completing each component of the discussion. Contact your course faculty if you have questions.
General Instructions
Use the practice problem identified in the Week 1 discussion and include the sections below. Select a practice change that is client-focused, specific, measurable, and related to your future role in advanced nursing practice. Nurse practitioner students must choose a practice change that impacts client care directly and avoid topics related to changes in public policy or law, such as full-practice authority.
Include the following sections:
Application of Course Knowledge: Answer all questions/criteria with explanations and detail.
Summarize your practice problem in 2-3 sentences.
Describe the evidence-based intervention you plan to implement to address the identified problem.
Write your practice question in standard PICOT format: In ________(Population), what is the effect of _________(Intervention) in comparison to _________ (Comparison) on _____________ (Outcome) to be completed over __________ (Time).
Identify components of your PICOT question where peer input would help refine those elements.
Integration of Evidence: Integrate relevant scholarly sources as defined by program expectationsLinks to an external site.:
Cite a scholarly source in the initial post.
Cite a scholarly source in one faculty response post.
Cite a scholarly source in one peer post.
Accurately analyze, synthesize, and/or apply principles from evidence with no more than one short quote (15 words or less) for the week.
Include a minimum of two different scholarly sources per week. Cite all references and provide references for all citations.
Engagement in Meaningful Dialogue: Engage peers and faculty by asking questions and offering new insights, applications, perspectives, information, or implications for practice.
Peer Response: Respond to at least one peer.
Communicate using respectful, collegial language and terminology appropriate to advanced nursing practice.
Professionalism in Communication: Communicate with minimal errors in English grammar, spelling, syntax, and punctuation.
Reference Citation: Use current APA format to format citations and references free of errors.

Poor glycemic control in adults with Type 2 Diabetes Mellitus (T2DM) is frequently associated with inadequate self-management behaviors, increasing the risk for complications and hospitalization. Evidence suggests that motivational interviewing interventions can significantly improve glycemic control and patient engagement in diabetes self-management (Berhe et al., 2020).
The evidence-based intervention I plan to implement is a structured diabetes self-management education (DSME) program combined with motivational interviewing. This approach goes beyond standard education by incorporating personalized teaching, goal setting, and behavior change strategies that actively engage patients. Motivational interviewing helps patients explore barriers, build confidence, and increase adherence to lifestyle and treatment plans, which have been shown to improve glycemic control. Perhaps most important will be a standard follow-up schedule to evaluate compliance and outcomes.
PICOT question:
In adult patients with Type 2 Diabetes Mellitus, what is the effect of a structured diabetes self-management education (DSME) program with motivational interviewing in comparison to standard diabetes education on glycemic control as measured by HbA1c levels to be completed over 3 months?
Peer input would help refine several components of the PICOT question. First, feedback on the time frame (3 months) would be valuable to determine if this is sufficient to observe meaningful changes in HbA1c levels. Second, peers could suggest whether the intervention should be more narrowly defined, such as specifying frequency or delivery method (e.g., in-person vs. telehealth DSME). Lastly, input on the outcome measure could help determine if additional outcomes—such as patient adherence, self-efficacy, or quality of life—should be included alongside HbA1c to provide a more comprehensive evaluation.
