This week, you will discuss how the Merit-Based Incentive Payment System (MIPS) impacts quality and safety.

Examine how advanced practice nurses can contribute to the development and refinement of MIPS quality measures to ensure that they are valid, reliable, and meaningful for clients and providers.

Wednesday Participation Requirement: Provide a substantive response to the graded discussion topic (not a response to a peer or faculty), by Wednesday, 11:59 p.m. MT of each week.
Total Participation Requirement: Provide at least three substantive posts (one to the initial question or topic, one to a student peer, and one to a faculty question) on two different days during the week.

A) Describe one practice area in which MIPS could be used to improve the quality of care related to your anticipated futurepractice setting. 
The Merit-Based Incentive Payment System (MIPS) is designed to improve patient outcomes across all healthcare settings. MIPS facilitates quality improvement in the management of chronic conditions, such as diabetes, within primary care. The system rewards providers to comply with evidence-based quality measures, including regular blood tests, diabetic retinopathy screening, and lifestyle counseling. Consistent monitoring, early intervention, and patient education are necessary for meeting these standards. These approaches support effective blood glucose control and reduce diabetes-related complications. By linking financial incentives to improved clinical outcomes, MIPS promotes continuous advancements in chronic disease management. Ultimately, both patients and care providers benefit from the incentives provided by MIPS.
B) Discuss two ways MIPS could positively impact the practice area
MIPS enables healthcare providers and organizations to preserve financial viability while boosting patient outcomes. The program ties a portion of Medicare payments to provider performance in quality, cost reduction, and the use of electronic health records. Providers who achieve high performance may receive positive payment adjustments, thereby increasing their Medicare reimbursement. These incentives encourage investment into quality improvement, care coordination, and technological advancements. As a result, clinical operations become more efficient and financially sustainable. Patient health outcomes improve as MIPS rewards providers for providing high-quality, patient-focused care, including prevention, chronic disease management, and coordinated care.

Advanced practice nurses (APNs) have extensive experience in direct patient care and can significantly contribute to the development and refinement of MIPS quality measures. Their clinical experience ensures that these measures remain current, reliable, and meaningful for both patients and providers. APNs advocate for patient-focused practice by participating in the ongoing evaluation and improvement of quality measures. Their involvement helps to guarantee that MIPS quality measures are evidence-based, relevant, and beneficial in routine clinical practice. Additionally, APNs can assess the reliability and validity of proposed MIPS measures by providing feedback on measure definitions and data collection methods.

Effective healthcare costs are incredibly important and even more so recognized since the pandemic. The previous models of healthcare which focused heavily on profit maximization may have heightened the focus on disease care and less on preventative care. Despite many efforts to lower costs, such as bundling payments, pay-for-performance, and limiting services, there is little evidence to demonstrate effectiveness and/or impact to communities†health and wellness.
Sometimes the question remains: “Does cost-effective care mean better healthcare?” According to Shenoy et al. (2019), quality care that is accessible, affordable, and provides health benefits to the population has been one of the overarching goals of many US health care programs and these programs include the Merit-based Payment System (MIPS), the Hospital-Acquired Condition Reduction Program, and the Hospital Readmission Reduction Program. Under the Merit-Based Incentive Payment System (MIPS), Medicare does closely evaluate costs. It is important to be able to evaluate findings from MIPS to the National Quality Strategy domains. Here, we are going to see clear views of outcomes which are incredibly important to healthcare providers.
