The opioid crisis continues to be a critical public health issue in the United States. According to the Centers for Disease Control and Prevention, approximately 105,000 people died from drug overdoses in 2023, with nearly 80,000 of those deaths involving opioids (CDC, 2024). Although there have been recent declines, opioid-related mortality remains significantly higher than pre-pandemic levels and continues to affect communities nationwide (CDC, 2025).
Treatment gaps persist, as national data indicate that millions of individuals require substance use treatment, yet only a fraction receive it. Notably, only about 17% of individuals with opioid use disorder receive medications for treatment (SAMHSA, 2025).
Untreated opioid use disorder is associated with an increased risk of overdose, infectious diseases such as HIV and hepatitis C, and co-occurring mental health disorders. It also leads to higher rates of emergency department visits, hospitalizations, and healthcare costs. Evidence shows that expanding access to medication treatment significantly reduces overdose deaths and improves patient outcomes.
Role of the Nurse Practitioner
Nurse practitioners are well-positioned to improve outcomes for patients with opioid use disorder through early identification, treatment, and ongoing support. They can incorporate routine substance use screening into primary care using validated tools, which enables earlier diagnosis and intervention.
Nurse practitioners can initiate and manage treatment with buprenorphine, provide counseling through motivational interviewing, and educate patients about overdose prevention, including prescribing naloxone when indicated. They also coordinate care with behavioral health providers and community resources to ensure continuity of care. Addressing social determinants of health, such as housing instability, transportation barriers, and medication access, remains an essential component of comprehensive care.
By treating opioid use disorder as a chronic and manageable condition, nurse practitioners can help reduce stigma and enhance patient engagement and long-term outcomes.
Rationale for Interest in the Problem
This problem is significant because opioid use disorder is frequently encountered in clinical practice and remains underdiagnosed and undertreated despite the availability of effective therapies. Addressing this issue supports the delivery of compassionate, patient-centered care and improves both individual and population health outcomes.
As a future nurse practitioner, I believe that focusing on opioid use disorder offers an opportunity to reduce preventable deaths and improve the quality of life for patients and their families.
