Patient falls have been a concern for probably as long as we have been providing a systematized approach to healthcare. Many Quality Improvement (QI) initiatives have been implemented to address falls, yet we remain concerned as it is a high priority to patient safety. As someone who worked in QI projects to determine new policies to address falls, I can tell you it often felt like a roller coaster. A great new idea was born, and enthusiasm followed a new intervention only to find the data did not support any change in fall incidences. This is an excellent example of why professionals working in healthcare should take a step further and identify root causes for falls specific to their work environment and any specialty care provided to patients. For example, falls on a pediatric floor are going to vary from falls occurring on a med surg unit caring for patients 65 and older with chronic disease(s). Large studies have explored existing research findings to determine common sources of falls, yet this is just scratching the surface. For instance, the WHO studied falls and found common sources to include surgical error (World Health Organization: WHO, 2023). To make a larger impact on issues, such as falls, providers may need to apply a systems approach to safety, such as processes, leadership, and policy are committed to patient safety.
